Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Require food employees to wash their hands using the procedure delineated in the Arkansas Rules and Regulations Pertaining To Food Establishments section 2-301. Food employee washed her hands in the three-compartment sink (corrected). Require food employees to use only designated handwash sinks for hand washing. Spray bottles with chemical product did not have a label with common name of the chemical. Chemical products were stored on top of refrigerator; separation from food, equipment, utensils, linens and single service. Label of chlorine used as sanitizer indicates air dry after applied; conditions of use. Noncritical violations: Prevent contamination of food or food contact surfaces (utensils) by other people using the kitchen. Clean tableware should be stored with face down to prevent incidental contamination.

Noncritical violations: Wire shelves in walk-in-refrigerator have an accumulation of grease and particles; clean at frequency necessary to prevent accumulation. This will help prevent incidental contamination of food. Prep-table interior is not clean.

Packaged food or food that is in bags or packaged for self-serve by the customers must have label with name of the food, list of the ingredients and name of major food allergens, amount and name/address of the place that it packaged, such as salad, sandwich, mix fruit and nuts. Noncritical violations: Food not in the original packages for use in establishment must be identified with the common name of the food, such as sugar, brown sugar, salt, flour. Nonfood contact surfaces of equipment is not clean. In food preparation area wall and ceiling is not clean; clean at frequency necessary to prevent accumulation and possible cross contamination.

Springdale Early Learning Academy 1268 Electric Ave., Springdale Critical violations: Raw eggs were stored above packaged ready-to-eat food. Different food in the refrigerator have temperature above 41 degrees; packaged cheese 45 degrees, packaged yogurt 44 degrees, Ranch dressing 50 degrees, cheese in plastic bag 48 degrees. Potentially hazardous food prepared or opened container held for more than 24 hours must be clearly marked to indicate the date or day by which the food must be consumed or discarded. Discarding ready-to-eat, potentially hazardous food/time/temperature control for safety food prepared onsite or opened commercial container held at 41 degrees for seven days. Food must be cooled to 70 degrees in two hours and down to 41 degrees in four additional hours. Noncritical violations: Supply paper towels at all handwashing sinks. The handwashing sink and aids and devices may not be used for food preparation or utensil washing, or a service sink or in any other manner other than handwashing. Wiping cloths shall be used as intended and in compliance with Regulations Pertaining To Retail Food Establishment. Store in a sanitizing solution or put them in the to be washed pile. Bucks of Asia 2141 N. College Ave., Fayetteville Critical violations: None Noncritical violations: Sink full of dirty dishes from evening before, left in sink by dish machine. Using bowls for scoops for bulk sugar and salt; the hand contact surface for these scoops are in contact with the food in the storage containers; need to use scoops that have handles or use a clean and sanitized scoop each time. Small ice machine has leak; using small container as a catch basin for leaking water; the container was full and overflowing onto kitchen floor; the container must be kept drained to prevent overflowing while in use. The ice machine needs to be repaired to prevent the water leak.

La Quinta Inn and Suites 720 E. Millsap Road, Fayetteville Critical violations: Unless using time as a public health control, potentially hazardous cold foods must be 41 degrees or colder. Milk in pitcher at 46 degrees at time of inspection. Noncritical violations: Food temperature measuring device shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperature. Buffet cooler and back refrigerator lacked thermometers to ensure temperature maintained 41 degrees or lower. Lacked quaternary ammonia test strips to ensure sanitizer solution is at 200 ppm.
Townplace Suites 5437 South 48th St., Springdale Critical violations: Boiled eggs lacked date to ensure they are used or discarded within seven days of preparation or opening commercial package. Noncritical violations: Food temperature measuring devices shall be accurate within 1 degree of the intended range. Cooler/freezer thermometers were both off by about 10 degrees. The wooden shelves in the walk-in are not 6 inches off the floor. Nonfood contact surfaces of equipment that are exposed to splash, spillage or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent and smooth material. Metal shelves in walk-in are rusty and no longer easily cleanable. The wooden shelves must be sealed so they are nonabsorbent and easily cleanable.

The Station 400 S. Gentry Blvd., Gentry Critical violations: None Noncritical violations: Some foods in the hot holding unit were not at 135 degrees or above; unit adjusted. Food employee not wearing effective hair restraints during food preparation. Storing wrapped biscuits in cardboard boxes. Food must be stored in containers that can be washed and sanitized regularly. Wooden floor in the drink walk-in cooler is no longer in good condition; paint has peeled off surface exposing raw wood. Unfinished wood is not an approved surface for food storage. Repair as needed to bring the storage area up to current code. Surfaces must be smooth, easily cleanable, nonabsorbent and durable. Paint is peeling off the surface of the frame surrounding the condensation unit in food walk-in cooler; surface is no longer smooth, easily cleanable, nonabsorbent and durable. Condensation leak in the drink walk-in cooler causing standing water to accumulate on the floor. Moldy shelves in the food walk-in cooler. Door frame between the drink and food walk-in coolers is falling away from the frame or missing, exposing drywall and wood; other areas look like possible mold growing on the surface. One chest freezer has duct tape holding the insulation panel in place. Duct tape is not an approved repair material. One chest freezer gasket is torn. Ceiling in the food walk-in cooler is falling.
