Reasons included improper separation of raw animal food in freezers and coolers; inside and outside of several containers with food in them that were not clean to sight and touch; several food items in all cold holding units with no date marks; very heavy build-up of grease on all cooking equipment in the facility; several areas of missing floor observed under equipment that is not just exposed absorbent, unsealed concrete; heavy accumulation of dirt, debris, insect carcasses, and grease on floors under and behind equipment, and live and dead roaches in the facility.
