The most serious types of blunders are still common on wards, with 312 in the 12 months to last March.
These so- called “never events” are deemed so dangerous by the Government that they should not happen under any circumstances. But 56 occurred in the last three months alone, the latest NHS England data reveals.
Health Secretary Jeremy Hunt has branded them “utterly shocking” and in June launched a campaign to end “never events”.
Life-changing bungles at 47 of England’s 150 NHS Trusts included a healthy kidney removed when wrongly assessed as an ectopic pregnancy.
One woman had a fallopian tube wrongly taken out after she was misdiagnosed with appendicitis. Nine people were given a wrong implant or prosthetic limb.
One patient had a feeding tube which was meant for their stomach put into their lungs, while 20 others had surgical equipment – in one case a hypodermic needle – left in them after routine operations.
The Patients Association said: “These errors are the tip of the iceberg. Not all trusts have honest reporting systems.”
NHS England said: “Every ‘never event’ is one too many. We’re committed to finding ways to eradicate them.”
